Our admissions team will work with you to explore the right payment options based on your needs, ensuring you get the best possible treatment.
In a PHP, patients live at home but follow an intensive schedule of treatment. Most programs require you to be on-site for about 40 hours per week.
Outpatient treatment offers flexible therapeutic and medical care without the need to stay overnight in a hospital or inpatient facility. Outpatient care typically offers a range of therapies and medical interventions individuals can attend alongside daily life.
Treatment for co-occurring mental health disorders, which affect the majority of individuals diagnosed, integrates care for mental health conditions with primary substance use. Personalized care plans are crafted to effectively address each condition simultaneously, optimizing the healing process.

What ever you do dont bring your loved ones to any of Eosis facilities. I was a contract nurse at the facility for about 6months where I witnessed other staff members acting as Nurses. These individuals dont have proper medical licenses (RN or LPN) are performing duties that require professional medical training and certification. These unlicensed staff members are reportedly making medical decisions and conducting patient assessment, which is beyond their scope of practice and potentially dangerous for patient safety. This situation raised significant concerns about the quaility of care being provided at this facility. The staff behind that sits in the nurses station are not nurses and they will not correct you if you call them a nurse. They love the title, but did not earn a degree. I had noticed that one of the TMA's that is what the staff are called not sure why the only thing that they know or can do is to pass medications. So I had noticed that one of the TMA's (not in any way trained to even be a medical assistant I do think they know CPR not sure) I as a nurse was reading a progress noted and had noticed that this particular TMA had signed her name with RN after it. I confronted her and told her that she could not do this and she came up with some excuse like "oh I didnt realize that I did that" RIGHT!!!. Multiple times I was the only nurse working with another TMA everytime a call came over the walki for a nurse to come and assist a patient in need she would run and grab her stethoscope and run over there finally I was so sick of it I just let her be the NURSE even though she was not. I think it is really sick that this company does not care if the staff their staff is lying about there credentials and letting all patients think that they are nurses. One day a new TMA that was 1 year out of recovery had just been hired there I belive he had worked there for a couple of months. He was working a double into the noc shift only him and another staff member both again no medical backround. A patient was sleeping on the couch across from the nurses station and the TMA states that he stopped hearing the patient and went to check on him and he was not breathing. This patient was only in his early 30's. ( this is a short version of the story there is much more to it such as falsifying documents and so on) I came into work the next morning once again I am the only nurse there and we had a Nurse manager (She was a nurse manager working on a restricted license by the way) I found out that this patient died so I went to look at the report from the TMA and let me tell you I was stunned. I could not belive it. He stated in his report that the first thing he did was called the Nurse manager at her home at about 4am. I asked the TMA why would you call the nurse manager dahhhhhhhh the first thing I would do is call 911 NOPE this was not done of course it was done after he called the nurse manager which took very critical time away from this patient. Honestly I could write a book on this place it should be shut down and the family members of this patient should sue the company for neglect. I also think the board should be notified. I watched everyone that morning come together and try and make late entries into this patients chart to cover there assess. So this place I would not even give it a star and to you COLLEEN DON shame on you!!! You know what is going on there and do nothing about it. You are a disgrace to the nursing field
