Situated along U.S. Route 30 in Fremont, Nebraska, this outpatient clinic delivers integrated primary medical and behavioral health care for children, adolescents, and adults. Services address mental health, emotional health, and substance use needs through coordinated outpatient care. The program treats attention deficit hyperactivity disorder (ADHD) and other neurodevelopmental disorders, anxiety, depression, trauma, post traumatic stress disorder (PTSD), grief, and life transitions. Coordinated Care For Everyone Care teams coordinate behavioral health with primary care to support whole person health. Psychiatrists, physicians, therapists, social workers, and substance use counselors provide assessments, medication management, and outpatient therapy. Providers address stress management, chronic illness impacts, and family concerns. Virtual visits extend access using secure video, supporting ongoing care while maintaining continuity with in person services. Access, Affordability, and Ongoing Support Appointments run Monday, Wednesday, Thursday, and Friday from 8 a.m. to 5 p.m., with extended hours to 7 p.m. on Tuesdays. Sliding fee options follow Federal Poverty Guidelines, and Medicaid and major insurance plans are accepted. Region funded support may cover services for eligible uninsured patients, with after hours assistance available on evenings, weekends, and holidays.
