Outpatient treatment offers flexible therapeutic and medical care without the need to stay overnight in a hospital or inpatient facility. Outpatient care typically offers a range of therapies and medical interventions individuals can attend alongside daily life.
Treatment for co-occurring mental health disorders, which affect the majority of individuals diagnosed, integrates care for mental health conditions with primary substance use. Personalized care plans are crafted to effectively address each condition simultaneously, optimizing the healing process.

Excellent experience and wonderful empathetic care.
I want to thank Carrie Daly, nurse practitioner. I have never met or been treated by such a caring person. She gave me hope and I really feel she wants the best for me. She goes above and beyond. We need more medical staff with her compassion! Thanks Carrie, you are wonderful. Along with Carrie are 2 physical/occupational therapists that I want to mention, Shannon and Katie. They have made me feel special. The workouts may be a little tough, but Shannon and Katie are so personable that we laugh and joke as I workout. I honestly look forward to their visits! Thanks Ladies, you are the best!
I’ve had a really positive experience with Bella Terra Elmhurst! The facility is always clean and well cared for. The staff are friendly and genuinely engaged with the residents. Overall, I feel confident and at ease knowing my loved one is receiving great care here.
Please if you love your family as much as I love my mom, do not use Bella Terra. Worst care ever. She is in such bad shape after being there. She had a fall there and and nobody seem to know what’s going on. She is coming home with infected bed sores, which she didn’t have to begin with. She’s Coming home without an air mattress, which I was told by the wound doctor that she needs. Can’t tell you how many times I told the care team what her baseline was and nobody seemed to write it down or try to get her Back to where she needs to be. This has been a nightmare!!!! Stay away from Bella terra!!!!
The team at Elmhurst is absolutely amazing with award winning service. The facility is always clean and feels like home.
The name Bella Terra means “Beautiful Land,” but this facility is far from that. When you first visit, the Director, Dimas, greets you warmly and seems very welcoming. However, once your loved one is admitted, that friendliness disappears — both from him and the staff. My 88-year-old father went there after surgery. He was weak, in pain, and diabetic. When he arrived, I reminded the staff about his diabetes, but they told me, “He gets whatever we give him.” They served him greasy Polish sausage, fries, and a chocolate brownie. When I told them he couldn’t have those foods, they argued with me, insisting he wasn’t diabetic — even though I’m the one who takes him to his doctors. Things only went downhill from there. He didn’t receive any of his medications — neither pain meds nor his regular prescriptions — because, according to the staff, the delivery from the pharmacy hadn’t arrived. Despite my repeated requests for them to follow up (over 8 hrs of requests) , they refused, saying he’d have to wait until the next day. After hours of pain and neglect, my father broke down — screaming, crying, hitting himself, and begging to go home or back to Elmhurst Hospital. The staff still refused to help, telling him to just wait. At 2 a.m., I received a call and rushed to the facility. They wouldn’t release him to me, claiming they had to speak with their doctor first & all was fine. During a video call with the doctor in the am (4am-ish), they said he could only be released to a hospital because he was “suicidal.” He wasn’t — he was simply in unbearable pain and said he’d rather die than continue being in pain. Finally 911 was finally called, the hospital staff immediately checked his vitals upon arrival — his blood sugar was over 500 (it had never been above 250 at home), and his blood pressure was dangerously high & they hospital quickly determined he was not Suicidal (He Loves His Life & Family). He was in worse shape than when he had first left the hospital. After hearing what happened, the hospital decided to keep him for rehab rather than send him back to Bella Terra. Please do your research and visit any rehab facility thoroughly before admitting your loved one. This place almost cost my father his life.
My daughter has been in Bella Terra almost 4 years. I can't say enough about the excellent care she had received. I would like to acknowledge Reneisha and Cheryl for going above and beyond. Mary Claire Mulvihill
I was impressed by how responsive the team was. Any time I called with a question, I got a clear answer right away.
