PHP: During the initial PHP, thirty one day treatment plan.
THERAPY; was the active therapist for Derek. In four weeks, Derek saw for a total of less than two hours. Unfortunately, this was a much needed treatment fo didn't receive. One on one therapy is critical during the first 30 days of treatment.
EMDR: During the initial interview session with Derek was instructed that he w 20 minute sessions. After researching EMDR, the standard length of a treatment sessic minutes. Unfortunately, Derek was in dire need of this treatment also, but did not receive this. LEGAL ASSISTANCE: As we instructed

OPS: Group was supposed to meet 5 days per week from 6-9pm. Apparently that feel apart due to the lack of staff and at times patience to attend the meetings. Apparently, there is a minimum amount of patience required to attend (3) before it is deemed as a therapeutic group. If not, the insurance would
not disburse funds.
1OP3: Since 10PS falled, IOP3 was sanctioned and the group met 3 times per week. Unfortunately, staff that were assigned to run the group were not always qualified. We have documentation where one of the staff falsified the documents in order for the insurance company to pay the fees.
DRUG THERAPY: Derek was prescribed with Suboxone and he was a self-pay for the prescription. The staff distributing the drugs in IOP were not certified professionals and at times would hand out Derek's neds to other patience. Major offense.
PR'OGRAM COMPLETION: Derek successfully completed the program in April 2023. He had asked for the er of completion and didn't receive it until June 2023. As part of the legal aid, that letter of pletion should have been submitted to the court in Florence, AZ. To date, we are still receiving cations from the court as nothing has been received on their behalf.

As an example, transportation. knew he did not have a car and the agreement was to pick him up for at least grocery shopping. er happened and he ended up walking rain or shine.This has been a disappointing experience for Derek and a long uphill battle fighting for the items that were included in his treatment but not received. It isn't enough that addicts are fighting a disease that has overtaken their minds, but adding this level of stress to a recovery program could only substantiate the extremely low success rate we see as a nation.
Ve are writing with expectation of reparation for the poor quality almost non-existent treatment that as precluded and sold to our family by Ascend Recovery. We are not without documentation, emails, at and phone calls from staff, family's and current and former patients. We feel that we should be compensated for services not received.
