Levels of Care
Outpatient treatment offers flexible therapeutic and medical care without the need to stay overnight in a hospital or inpatient facility. Outpatient care typically offers a range of therapies and medical interventions individuals can attend alongside daily life.
Treatment for co-occurring mental health disorders, which affect the majority of individuals diagnosed, integrates care for mental health conditions with primary substance use. Personalized care plans are crafted to effectively address each condition simultaneously, optimizing the healing process.

I spent 25 years going to this pain management clinic..... Made one innocent mistake which they never had the Guts to ask me about to my face they sent me a termination letter.. this is the worst pain management clinic in Indiana.. Christina was my nurse practitioner I thought she was better than that... I thought wrong... I give this place zero Stars!!!!!

I’m very happy with the care I receive. They always get back to me quickly and consistently go above and beyond.

I swear to God I've never had the best treatment and been taken care of so good. I've been going here for 5 years and Elisa Graves has helped me with any and everything that I've ever needed from stacks of FMLA paper work to prior authorization's. She's the best I've ever met in the medical field. Thanks Elisa you rock girl

I absolutely love Dr. Kazmi. I've been looking for a physician who actually cares enough to listen and he's one of those.

This place was great for years until they became scammers at the beginning of 2026. Idk what happened to their office & billing department but they are so unhelpful, can't tell you anything, & just demand money. I used to look forward to going here & going to group, now I dread going & will be leaving so I don't lose my mind

This place needs to be ashamed of itself. I stopped attending this physicians office almost ten years ago because they have 1-2+ hour wait times even if you're the very first appointment. I thought I would give them a try again today. They left me and another woman who's appt was also at 8 am sitting in the waiting room, having 'accidentally' checked us both in and then right back out. I waited 50 minutes before I left. The woman I was sat with in the waiting room said this exact same thing happened to her yesterday. Her appt was at 8 am and they didn't try to see her until 10 am. The clinical manager did reach out to me, but when I demanded my money back from my unused co-pay, she insisted that she'd have to reach out to billing. Save yourself a massive amount of headache and time. Seek care elsewhere. I second the previous poster. Go see Dr Ramirez at CMC.

This place is very unprofessional I had appointment set up for 2 pm for my mentally ill mother and had to wait till 4 to be seen ,while waiting I could hear the front desk workers making fun of my mother and others waiting in the waiting room to the point i had to say something because my mother didn't want to deal with them anymore . After we met with her doctor they still didn't wanna give her all her medicine .

I scheduled an appointment to be seen on 01/29/2026 at 2:15. I was told that I wasn’t on the schedule for today, that I was scheduled for 01/30/2026 at 2:30 instead. Front desk said they would check to see if Amy could still fit in. Front desk tells me “she can see you today at 3” & then Dr. Amy Martin steps to the window, while I am in a waiting room with other patients to tell me “I will see you but I will not refill your xxxxx prescription” giving out my medical information to the lobby to hear. Even though I was upset and actually offended because 1. I’m not and never have been an addict, I do not go to this office for pain management. This provider was listed by my insurance 7 years ago, and I’ve been with them since. I am seen for a MEDICAL condition that causes me to have panic and anxiety attacks. I have been treated for this for almost 7 years . I am not a pill seeker, I take my medication as needed and always have. I accepted the appointment because my original doctor was booked weeks out and I needed my refills . I was called back and seen my Amy, she said she would only call in one of my medications not both, I said that’s fine, but, what am I going to do when I have no refills remaining and I come to be in need of my medication she’s refusing to fill. She said call the office and have my original physician handle it. Which leads me back to square one because, he is booked out. I then asked her to chart that she is denying my refill and why. She refused to do so. I walked out to the front to schedule my follow up appointment and as I’m in line I hear her telling the front desk “MY NAME” “is not to be scheduled on my schedule again, she is only to be seen by Dr. ***” again, having conversations about my medical history for the waiting room to listen. For someone that is not seeking or in pain management, or struggles with addiction I can only imagine how someone in that position is being treated by this provider. 1. She violated HIPPA when she announced my medication to the waiting room. 2. I have an actual disability that I was diagnosed with outside of this office, with a specialist, that NORTH CLARK REFERRED ME TO & I seek help for at this location and instead of providing me with the help I need for my treatment, I was discriminated against and denied my medication . I couldn’t care less if management gets back with me, because I will make sure to escalate this to the highest levels possible with OCR and the medical board.
