A collaboration between Isaiah House and Lexington Rescue Mission, this Community Health Center provides essential behavioral health and medical care to Lexington area community members. They offer job assistance services, counseling, immunizations, chronic disease management, gender-specific health services, medication-assisted treatment for opioid addiction, and more. Personalized Care and Support Their case management team helps coordinate services, create personalized care plans, and connects individuals to necessary resources to ensure that all needs for well-being are met. The Community Health Center also has a peer support program that pairs individuals with someone who has faced similar challenges and can provide guidance and encouragement. Their peer support specialists are specially trained and are ready to support people in whatever they’re walking through.
