Shocking Misdiagnosis and Falsified Medical Records - A Warning to All
In July of last year, I suffered a work injury. I underwent surgery in October and have been out of work for seven months, without salary or insurance.
This forced period of reflection led me to address some unhealthy patterns in my life. Having a history of childhood ADHD, I sought professional help for potential diagnosis and medication to manage impulsivity and reactivity. I was referred here. My psychiatrist appointment was set for March 5, 2025, after waiting since late January.
The 15-minute video call with the psychiatrist was deeply concerning. I briefly mentioned struggles with road rage due to slow drivers and a recent confrontation in a grocery store where I responded to prolonged harassment. Based on these limited interactions, I was diagnosed with "rage" and, incredibly, bipolar disorder at the age of 60, and prescribed antipsychotic medication.
However, the real shock came when I reviewed the psychiatrist's notes. The information documented was entirely fabricated. The notes stated: "She has dynamic risk factors for dangerous behavior including ongoing mood symptoms, recent psychiatric hospitalization, recent incarceration, unemployment, lack of stable housing, legal problems, substance use, lack of insight."
This is utterly false. I have never been hospitalized for psychiatric reasons, incarcerated, or struggled with substance abuse. I have owned my home for eight years. My unemployment is directly related to my recent surgery following a work injury. To suggest I lack insight is ironic, given the blatant inaccuracies in their records.
I am writing this to warn others: meticulously review your medical records. These documents can be subpoenaed in child custody cases, by employers, or in legal proceedings. False information, like what I encountered, can have devastating consequences.
When I contacted the center to request the removal of these false records, they simply removed them from the patient app. I have no doubt the records still exist within their system.
Therefore, I urge everyone to be proactive and vigilant about their healthcare. Become the steward of your own medical information. Pay close attention to the clinical notes provided after each appointment.

They replied to this review, but you’re not able to reply back to them. I did dial that phone number and press the number six and it doesn’t exist. Pressing number six doesn’t exist. So I called back and pressed one and that person didn’t even know what I was talking about when I asked for the rights office.
This is an actionable case. So I suggest that your organization get their act together.
